If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible. Accepted Insurance Carriers:
As a courtesy, we will file insurance claims for you; however, we accept assignment of benefits beginning with the second appointment. (There are a few exceptions to this rule.) In order to process your insurance claims, we need your insurance card and the subscriber's identification number, many times this is the social security number. Please note that insurance benefits are dictated by a contract between an employer and an insurance "carrier" or company. We are not party to that contract. The non covered services are not based on what you need or want, but are based strictly on the contract with the insurance company. Insurance companies have no interest in your health.
