As
a courtesy we will file any insurance claims for
you, however, we only accept assignment of insurance
benefits after your second visit. We do require
your portion to be paid at the time of service. The
balance is your responsibility whether your insurance
company pays the claim or not. In order to
bill your insurance company we need you to provide
us with all your insurance information, such as your
insurance card and the subscriber's social security
number. Your insurance policy is a contract
between you and your insurance company. We
are not party to that contract. Please be aware
that some and/or all of the services provided may
be "non-covered" services and not considered "reasonable" and
necessary under your dental insurance plan. Our
practice is committed to providing the best treatment
possible for our patients and we charge what is usual
and customary for our area. You are responsible
for payment in full regardless of any insurance company's
arbitrary determination of usual and customary rates.

Full
payment is due at the time of service. We accept
cash, checks, VISA, MasterCard and American Express.
For
any treatment plan over $1000, we do offer a 5% discount
on full payments that are made with cash or check.
For
your convenience, we do offer an extended payment plan
with an outside financing company. It does require
prior credit approval.
